If your child, or you as a young person, have been invited to take part in our Youth Work Programmes in the ways listed below from 1st January 2026 to 31st December 2026, then a consent form will be required.

This form collects consent from parents or guardians for the forthcoming opportunities.

Before you attend an activity where a consent form is required, please complete and submit here.
Please submit separate forms if you have more than one child who will attend.
