The Autism Health Passport is a free document designed by Torbay’s Autism Partnership Board and Autism Ambassadors. It helps autistic people share important information about themselves, their health needs and their communication preferences with health professionals.

How to access

Download the free Autism Health Passport from the Torbay Council website and complete it. The passport can then be taken to medical appointments and shared with health professionals.
