If you have received a draft EHCP you need to complete and return the form that you were sent alongside the draft EHCP within 15 calendar days.

The form asks:

• whether or not you agree with the contents of the EHCP
• for you to let the LA know what school you would like to be named on the final EHCP. Please see Choosing and naming a school (or setting) in an EHCP for more information.
