A request for an Education, Health and Care (EHC) needs assessment (EHCNA) is a process where information is collected from the parents, child or young person and professionals. This then allows us to gain a clear picture of the education, health and care needs of the child or young person. Depending upon the level of needs and provision required identified through the EHCNA this may result in an Education, Health and Care plan (EHCP) being issued for a child or young person.

EHCNA requests for schools and other educational settings:

• Early Years settings – EHCNA/AppB – Request from Early Years setting for the LA to consider statutory assessment
• Educational settings – EHCNA/AppB – Request from educational setting (for primary aged children) for the LA to consider statutory assessment
• School or College with PFA – App B EHCNA – Request from educational setting (for secondary and above aged children and young people) including Preparing for Adulthood section for the LA to consider statutory assessment
• Views of the child or young person – App F including One Page Profile – Views of the child / young person for EHC assessement.
• Views of the young person – YP’s questionaire template – Views of the child / young person for EHC assessement.
