Forms to request an EHC needs assessment – requests made by a parent of a child or young person under 16 years of age

A request for an Education, Health and Care (EHC) needs assessment (EHCNA) is a process where information is collected from the parents, child or young person and professionals. This then allows us to gain a clear picture of the education, health and care needs of the child or young person. Depending upon the level of needs and provision required identified through the EHCNA this may result in an Education, Health and Care plan (EHCP) being issued for a child or young person.

EHCNA requests for parents to use:

When submitting a request for assessment for your child it is best to include both the your application, using the “Parents – EHCNA/App A” form above, and your child’s views, if primary aged, using the “Views of the child or young person – App F including One Page Profile” form above, or your young person’s views, if secondary or above aged, using the “Views of the young person – YPs questionnaire/alternative App F” form above.
