An Education, Health and Care (EHC) needs assessment (EHCNA) is a statutory assessment is a process where information is collected from the parents, child or young person and professionals. This then allows us to gain a clear picture of the education, health and care needs of the child or young person. Depending upon the level of needs and provision required identified through the EHCNA this may result in an Education, Health and Care plan (EHCP) being issued for a child or young person.
When submitting a request for assessment for your child it is best to include both the your application, using the “Parents – RSA/AppA” form above, and your child’s views, if primary aged, using the “Views of the child or young person – App F including One Page Profile” form above, or your young person’s views, if secondary or above aged, using the “Views of the young person – YPs questionaire/alternative App F” form above.
