Forms to request an Education, Health and Care needs assessment

When submitting a request for assessment for your child it is best to include both the your application, using the Appendix A form above, and your child’s views. If primary aged, using the Appendix F – views of the child or young person form above, or your young person’s views.

Request by a parent of a child or young person under 16 years old

An application for an Education, Health and Care needs assessment (EHCNA) is a process where information is collected from the parents, child or young person and professionals. This then allows us to gain a clear picture of the education, health and care needs of the child or young person. Depending upon the level of needs and provision required identified through the EHCNA, this may result in an Education, Health and Care plan (EHCP) being issued for a child or young person.

Request by a young person over 16 years old

An application for an Education, Health and Care needs assessment (EHCNA) can be made by a young person over the age of 16 but under 25 using the Appendix F – child / young person alternative views form above.

Request made by a school or other educational setting

An application for an Education, Health and Care needs assessment (EHCNA) for a child or young person may also be made by a person acting on behalf of a school or post-16 institution (with the knowledge and agreement of the parent or young person.
