The law says that if your young person is age 16 or above, it is up to them whether or not they wish to request an assessment for an EHCP. Providing you have consent from your young person, parents are also allowed to submit a request for assessment on their behalf.
When a young person submitts a request for assessment, they should use the “Young People – RSA/App A and F” form above. If you are the parent of a young person, and your young person has consented for you to also provide additional information or submit a request on their behalf you may also want to submit the “Parental – RSA/AppA” form above accompanied by the “Views of the young person – YP’s questionaire template/alternative App F” form above, unless your young person has also submitted the “Young People – RSA/App A and F” form above.
